PLEASE BE ADVISED that Medical Qi Gong, by and through Venus Sabay, is not a medical treatment nor is it intended to replace traditional medicine, but rather to complement it. By participating in live-stream classes you are agreeing to the following: I recognize that Qi Gong involves physical movement or exercise; I am fully aware of the risks and hazards in performing physical movement and exercise and agree that it is my responsibility to work within my own personal limits.

I understand that it is my responsibility to consult with a physician concerning and prior to my participation in any movement or exercise. I hereby acknowledge that I have no medical condition(s) that would prevent my participation in Qi Gong instruction.

I assume full responsibility for any risks, injuries, and/or damages, known or unknown, which I might incur as a result of my participation in Qi Gong instruction with Venus Sabay. In consideration of being permitted to participate in Qi Gong instruction, I expressively waive and hold harmless any claim I may have against Venus Sabay as a result of participating in Qi Gong instruction.

I, my heirs or legal representatives forever release, waive, discharge and covenant not to sue Venus Sabay and any representatives for any negligence, injury or claims whatsoever.
